SUBPOENAED RECORDS:
The Albany Fire Department looks forward to assisting you with your subpoena request. Please understand that there are “reasonable” fees associated with this request for records and they are as follows:
- Processing Fee of $24.00 per hour per person, or $6.00 per quarter hour or fraction thereof
- Actual postage charges
Additionally, there are secondary fees for the printed/faxed documents themselves:
- 10 cents per page for standard sized documents 8-1/2 x 14 inches or less)
- 20 cents per page for copying of documents from microfilm
- Actual costs for oversized documents
- There are no additional fees for emailed digital files.
The costs must be paid no later than simultaneously with actual delivery of the records. (Evidence Code § 1563(b)(2).

FIRE INCIDENT REPORTS:
Fire Incident Reports are available through the Fire Department Records Department. While a written request is not required, public record requests should be in writing so that Fire Department staff can more accurately identify the records sought and process the request more efficiently. Please be as specific as possible when making your request; include dates and times (if applicable), your relation to the property/address, and any details regarding the information you are requesting. For your convenience, these requests may be made by any of the following methods:

What you will need to request records:
- HIPAA Authorization to Release Medical Records (If requesting party is not the patient)
- Valid I.D. (California Driver's License, California I.D., Military I.D. or U.S. Passport)
- If the patient's signature is not available, legal documentation such as a subpoena, power of attorney, or letter of representation is required and must accompany the request.
